This content is for educational purposes only and does not constitute medical advice, diagnosis, or treatment. MOTS-c is a prescription peptide. Speak with a licensed medical provider before beginning any peptide protocol.

MOTS-c Before You Start: The Critical Protocol Facts

  • MOTS-c is a mitochondria-derived peptide that regulates cellular energy metabolism, making it mechanistically distinct from every other peptide in clinical use today.
  • The Perfect B protocol doses MOTS-c at 5mg (25 units) per injection, once every 5 days, for a total of 4 injections over a 20-day cycle.
  • Injections are timed at night, at least 2 hours after the last meal, to align with the body’s overnight metabolic and mitochondrial repair processes.
  • A minimum 4-month rest period follows each cycle, with a maximum of 3 cycles per year.
  • MOTS-c effects are cumulative, building across all 4 injections and the weeks that follow, not from any single dose.

What Is MOTS-c and Why Is It Different From Other Peptides?

A Peptide Encoded by Mitochondrial DNA

MOTS-c is a 16-amino acid peptide encoded not by the nuclear genome but by mitochondrial DNA. Virtually every other peptide in clinical use originates from nuclear DNA. The mitochondrial origin means MOTS-c acts at the cellular energy level in a way no GHRH analog, tissue-repair peptide, or cognitive peptide does. Because safe dosing depends on medical oversight, see how medically supervised peptide therapy at Perfect B in Doral works and how each protocol fits into a complete, monitored plan.

A 2015 Cell Metabolism study that first characterized MOTS-c as a mitochondria-derived regulator of metabolic homeostasis capable of reducing obesity and insulin resistance in animal models was foundational in establishing this peptide class. The finding shifted understanding of how mitochondria communicate beyond energy production, signaling through circulating peptides that reach distant tissues.

MOTS-c acts inside the mitochondria to regulate energy production and optimize cellular metabolism.

How MOTS-c Differs From Growth Hormone and Cognitive Peptides

Growth hormone peptides like CJC-1295/Ipamorelin, which requires daily injections and stimulates pituitary GH release to support sleep, muscle, and fat metabolism, operate through a hormonal axis. Cognitive peptides like Semax act on neuropeptide receptors. MOTS-c does neither. It works inside the cell, influencing how mitochondria process fuel and respond to metabolic demand. This distinction matters when evaluating a patient’s candidacy and setting expectations for the protocol.

MOTS-c Dosage Protocol: 5mg Every 5 Days for 20 Days Then 4 Months Rest

Our protocol uses a fixed dose of 5mg (25 units) per injection throughout the full cycle. There is no ramp-up phase and no dose escalation. You start at the full therapeutic dose from the first injection and maintain it for all 4.

Each MOTS-c dose is fixed at 5mg, administered every 5 days for a total of 4 injections per cycle.
ParameterPerfect B Protocol
Dose per injection5mg (25 units)
Number of injections4 per cycle
FrequencyOnce every 5 days
Cycle duration20 days
Injection timingNight, 2+ hours post-meal
Rest between cycles4 months minimum
Maximum cycles per year3

The 5-day spacing between injections reflects the pharmacodynamics of how MOTS-c signals at the mitochondrial level. Injecting more frequently does not accelerate the response and is not part of any protocol we prescribe at our Doral clinic.

The Day-by-Day Breakdown

Each MOTS-c cycle consists of exactly 4 injections spread across 20 days, one every 5 days:

DayAction
Day 1Injection 1, 5mg (25 units)
Days 2 to 5Rest, no injection
Day 6Injection 2, 5mg (25 units)
Days 7 to 10Rest, no injection
Day 11Injection 3, 5mg (25 units)
Days 12 to 15Rest, no injection
Day 16Injection 4, 5mg (25 units)
Days 17 to 20Cycle complete, begin 4-month rest
The MOTS-c protocol follows a 20-day cycle with 4 injections spaced every 5 days, allowing time for cellular response between doses.

Why Only 4 Injections?

Four injections over 20 days is a substantially lower frequency than any daily peptide protocol. This is by design. MOTS-c does not produce a dose-dependent linear response where more injections yield proportionally better results. The cellular signaling cascade it initiates requires time to propagate, and the 5-day gap is recovery time at the mitochondrial level. Patients sometimes ask whether a 5th injection would extend the benefit. It would not. The protocol is complete at 4, and the 4-month rest that follows is as important as the active cycle itself.

When to Inject MOTS-c: Nighttime, Fasted State Only

Inject at night before going to bed, at least 2 hours after your last meal. MOTS-c influences mitochondrial signaling pathways that interact with the body’s fasted-state metabolic processes. A post-absorptive state ensures digestive demands are not competing with the peptide’s cellular signaling, allowing for cleaner mitochondrial activation.

This is the opposite timing from cognitive peptides like Semax, which we administer in the morning to take advantage of its focus-enhancing effects during active hours. MOTS-c belongs to the overnight protocol category. Do not inject it in the morning or immediately after a meal.

What to Do If You Miss an Injection Day

If you miss a scheduled injection day, apply the dose when you can and count 5 days forward from that new date. Do not attempt to return to the original calendar. The 5-day interval is what matters, not the calendar day. Shifting the schedule by a day or two has no meaningful impact on cycle outcomes.

How to Reconstitute MOTS-c at Home

MOTS-c is shipped as a lyophilized (freeze-dried) powder and must be reconstituted with bacteriostatic water before injection. The process takes under 5 minutes and follows the same technique used for all injectable peptides we prescribe in Doral.

  1. Clean the rubber tops of both the MOTS-c vial and the BAC water vial with an alcohol swab
  2. Draw 0.5 mL of bacteriostatic water into a 3 mL syringe
  3. Inject the BAC water slowly into the MOTS-c vial, angling the stream down the side wall rather than directly onto the powder
  4. Gently swirl the vial until the powder is fully dissolved, do not shake
  5. Draw 25 units from the reconstituted vial into an insulin syringe for injection
  6. Store the reconstituted vial in the refrigerator between doses
Reconstitute MOTS-c by adding bacteriostatic water, injecting along the vial wall, and gently swirling, never shaking, to preserve peptide stability.

Injection Site and Technique

The injection is subcutaneous, into the abdomen. Gently pinch a small fold of skin and insert the insulin needle at a shallow angle. Because MOTS-c involves only 4 injections per cycle, rotate to a different quadrant of the abdomen with each dose to minimize localized tissue irritation. Dispose of all syringes in the sharps container provided with your kit and bring it to the clinic for proper disposal.

Storage After Reconstitution

Store the reconstituted vial in the refrigerator between injections. Do not freeze. A properly stored reconstituted MOTS-c vial remains stable for the duration of your 20-day cycle. Place it in the black protective case provided before returning it to the refrigerator after each dose.

How Many MOTS-c Cycles Can You Do Per Year?

Perfect B prescribes a maximum of 3 MOTS-c cycles per year, with a minimum 4-month break between each cycle. With a 20-day active period and 4 months off, the total annual protocol commitment is 60 active days spread across the year.

ParameterDetail
Active cycle length20 days (4 injections)
Minimum rest between cycles4 months
Maximum cycles per year3
Total active days annually at max60 days

The 4-month rest is not optional. Studies on mitochondria-derived peptides and their role in regulating long-term metabolic adaptation consistently point to off-cycle consolidation as a critical part of the protocol. The cellular adaptations initiated during the active phase continue to develop during the rest period. Cycling too frequently does not compound the benefit. It diminishes it.

What to Expect: Energy, Recovery, and Metabolic Changes

When Do Results Start?

MOTS-c does not produce acute effects after a single injection. The response is cumulative, building across the 4 doses and continuing into the weeks after the final injection. Most patients at our Miami clinic begin noticing changes by the second or third injection, with the clearest effects appearing 2 to 4 weeks after the cycle ends.

  • Improved energy levels: Reduction in afternoon fatigue and improved stamina during exercise, particularly in patients who were already physically active before starting.
  • Better metabolic efficiency: Some patients report favorable body composition shifts over multiple cycles, especially when combined with consistent training and nutrition.
  • Faster recovery: Reduced post-exercise soreness and quicker return to baseline after high-intensity training.
  • Increased physical resilience: A general improvement in how the body handles sustained physical and metabolic stress in the weeks following the cycle.

Who Responds Best to MOTS-c?

The patients who report the strongest response are those who are already active, eat well, and are looking to optimize cellular energy output rather than compensate for lifestyle gaps. This peptide amplifies a working foundation. It does not replace one. Patients focused on long-term metabolic health sometimes pair MOTS-c with GHK-Cu, a copper peptide that targets tissue repair, collagen production, and anti-aging at the cellular matrix level through a completely separate mechanism. The two protocols do not overlap and can be run in adjacent cycles under provider guidance.

How Perfect B Evaluates MOTS-c Candidates

Before prescribing MOTS-c, our providers review your lab results and health history to confirm this is the right protocol for your goals. We do not prescribe it to every patient who requests it. The mitochondrial energy pathway MOTS-c targets responds differently depending on the patient’s metabolic baseline, and we want to confirm the conditions where you will see meaningful results.

MOTS-c fits patients who:

  • Are already physically active with a consistent training routine
  • Want to improve metabolic efficiency, endurance, or recovery at the cellular level
  • Prefer a low injection burden, 4 shots per cycle rather than daily injections
  • Are looking for a protocol that complements, not replaces, existing lifestyle habits

For a broader look at every peptide protocol we offer, the Perfect B peptide therapy page covers all protocols available at our Doral, FL clinic, including what each one targets and how to get evaluated.
